Last year was the first year | had applied for this supplement and a friend helped me to complete the form. Unfortu- nately my friend has recently mov- ed to Vancouver Island, so she won't be able to help me this year. My English is not very good when it comes to writing and fill- ing in forms and I am not sure about some of the information they need on this application. Is there anywhere [ can get help? Those who need to make a first application should call Health and Welfare Canada at 666-3042 for an appointment. You will also need to complete your 1989 income tax return because you will need those figures for your GIS form.
