West Vancouver Police charged the driver with failing to maintain his brakes and making a false brake inspection report. Police won’t release the name of the driver. who works for Vancouver-based Payless Paving. Motor vehicle inspector Jay Northcott said four of the fully-loaded truck’s six brakes were out of adjustment. Northcott believes the driver’s speed was too fast to make the turn because the faulty brakes weren’t able to slow the truck to the proper speed.
