office is located in the North
Vancouver Court House building
at 200 East 23rd St. Their hours
are from 10 a.m. to 3 p.m. on
Mondays, Wednesdays and Fri-
days. The telephone number is
660-1268.
Your baby should be registered
no later than 30 days after the
birth by filling out the registration
of live birth form and returning it
to the district registrar.
Normally, both parents com-
plete the form. If only one pareni
is aole to register the birth, that
parent must complete a statutory
declaration form explaining why
the other parent has not signed.
The declaration form is located
on the czeverse of the birth regis-
tration form and must te signed
in the presence of a person with
legal authority to take affidavits,
such as a notary public. DOVS
office staff can perform this ser-
vice free of charge.
If, for some reason, a child’s
birth has never been registered,
the DOVS office can provide in-
formation using the delayed regis-
tration process.
Before a birth can be registered,
a mame must be chosen. Parents
are free to choose any names for
their child as long as it includes a
given (first) name and a surname
(family name). Middle names are
optional.
When using hyphenated sur-
names remember that they cannot
have more than one hyphen.
The DOVS publishes a booklet
titled ‘‘Babies’ Most Chosen B
