It was noted that there was an obvious neglect of the Industrial Health and Safety Regulations (IHSR), because the crane had no back-up - mirror or warning device. The jury recommended “that after the determination of a satisfactory training - adequate program and of a testing program all new crane operators be licenced.” Other recommendations included; making new workers aware of safety regulations, posting of safety regulations, and attaching back-up safety devices to mobile equip- ment. A similar recom- mendation was made following an inquest, also held in August, into the death of another industrial worker in North Vancouver. Larry Quaife, aged 18, was electrocuted and another worker was badly injured, when a crane came in contact with high voltage wires.
