How We'll Work: A Team Effort The key to moving health care "Closer to Home" is team work. Now, instead of individual departments, (like Community Health Nuring) we're building six multidisciplinary teams representing specific regions. These teams will be pioneering a proactive stance of health and wellness for each neighbour- hood! Teams consist of nurses, health inspectors, physiothera- pists and social workers, working to meet the needs of each area. If your team doesn’t have the answers to your questions, they'll work together to access the information from any of their countless community resorces. The Six New Teams: 1. W. Vancouver 2. N. Van. This could help eliminate stresses and help build on-going health relationships. ¢ With health care "Closer to Home", fewer trips to the hospital are needed. FAMILY HEALTH SERVICES: We handle services like home nursing care, rehab, long term care assessment and placement, community nursing in schools and other key locations, child and adolescent mental health services, and even specch and language therapy for preschoolers! Part of our job also includes dealing with all of the licensing of child care centres, group homes, and facilities for our frail and disabled community members. ENVIRONMENTAL HEALTH SERVICES: Our broad range of services extends to our public health inspectors who insure we have clean restaurants and food suppliers.
