This was obviously a chronic condition and possibly might have been aggravated by his pawing at the area, but it. wasn't from fighting. The condition was a fistula or a ““~passage between the outside skin and a body cavity - in this case a facial sinus. The sinuses are cavities in the skull which make it much lighter than “if there were more solid bone. The sinuses can become infected through the nostrils. When the facial sinus becomes plugged up with pus it breaks out through the facial bone and skin and discharges through an opening about 4% ‘inch below the eye. A course of antibiotics will often clear up this infection without much trouble. In other cases we have to maintain the patient on a continuous medication - preferably tetracycline - in small daily doses. In the old frail dog this may be preferable to surgery. Where the medicinal treatment fails we give an anaesthetic then drain the pus into the mouth by removing the molar tooth which has its root abutting into the facial sinus cavity. This is the largest of the molars and is called the carnassful tooth. It may be difficult to remove as_ its triple root makes it awkward to loosen with elevators. If the hole that is made into the sinus by removal of the tooth later closes up, the fistula may recur. Mrs. Bagshaw wanted to have the antibiotic treatment which proved quite ef- fective. A high dosage of tetracycline for 5 days followed by 50 mg. daily continuously cleared it up. I see on the record card that there has been a refill of her prescription three times over the past 6 months so everything is probably going along well.
